Registration for South Bruce Minor Hockey Association for the 2015-16 hockey year is being held on the following dates:
Wednesday June 10/15 from 7-9 pm at the Teeswater Ball Diamond Pavilion
Wednesday June 17/15 from 7-9 pm at the Mildmay Ball Diamond Pavilion
Saturday June 20/15 from 9-12 noon at the Mildmay Ball Diamond Pavilion.
Please note in case of inclement weather, registration will be held in the arena lobbies.
Below is the registration form. Please complete online (its a fillable PDF) and then print off and bring to registration. Please note that you will have to manually add the totals (registration + fundraising fee) for the total at the bottom of the form. The fundraising fee can be included with the registration cheque and then a separate cheque for the volunteer bond and jersey deposit (if applicable).
Please be advised any parents of new players under the age of 17 in the South Bruce Minor Hockey Association must also complete the Respect in Sport program. Details can be found under the tab "Respect in sport" in the Registration section.
